Reserves Request Form

This form must be submitted at least one week prior to assigning these materials to your students. Materials requested for reserve use are removed from the shelves at the end of each academic term. A new form must be completed for each term. Please note that all material is subject to copyright restrictions, and copyright clearance is the responsibility of the instructor.
